SAMNet Vets Program
For many veterans, sight loss is the new battleground. Whether blinded as the result of war, or losing vision later in life, there are numerous obstacles to be conquered. Though the challenges are many, we know that our veterans can face these challenges with the same bravery and dedication they demonstrated in service to our country.
While there are no words or deeds that could ever adequately express the depth of our gratitude, we want to do our small part to express our appreciation for everything our blinded veterans have given in service to our country. As a result, Serotek is happy to extend lifetime access to the System Access Mobile Network, (SAMNet), to all US Veterans who qualify for services from the VA Blindness Rehabilitation Services division. How do I know if I am eligible for the SAMNet Vets program?
If you are a veteran who has served on active duty with the U.S. military and you are now eligible for services through the VA Blindness Rehabilitation Services division, you qualify for the SAMNet Vets program. You will need to work with a Visual Impairment Services Team (VIST) professional in order to complete the paperwork verifying your eligibility for this service. How do I apply for the SAMNet Vets program?
To apply for the SAMNet Vets program, here’s what you need to do. Note that incomplete documents will be discarded.
Complete and submit the online SAMNet Vets application form, taking care to fill in all fields. This form will automatically create a SAMNet account which is active for 30 days, pending the submission of the signed paperwork. If a Serotek account for the veteran already exists, please enter either user name or account number in the appropriate fields on the form. If you believe you may already have a Serotek account but aren’t sure, you may call Serotek customer service at (650) 249-1000 and a representative will be happy to assist you.
Once the form has been submitted, an email will be sent to the VIST professional listed on the form, in order to verify eligibility. The VIST professional should print the form using the link found in the email. The form should be faxed to 612-392-3881, or emailed as a PDF document to info (@) serotek.com. Please include a cover letter on your organization’s letterhead stating that the enclosed documents are to be used for participation in the SAMNet Vets program. The documents will be examined upon receipt and if all is in order, the SAMNet account for the veteran will become active for life.
